Case Management
The Case Management Department provides comprehensive care coordination to patients with the goal of improving individual health status and social well-being.
We are Community Partners with The PA Department of Public Welfare. The case manager will utilize the web-based program Compass to determine eligibility and apply for state programs; such as Medicaid, Select Plan for Woman, food stamps, cash assistance, and the Pennsylvania Low Income Home Energy Assistance Program (PALIHEAP).
Case management will aid the uninsured and underinsured in obtaining free or low-cost medication through a variety of web-based Patient Assistance Programs as well as track and re-order prescriptions for our patients. Our department will assist with Social Security’s Medicare and Medicare Part D programs.
Because of our collaboration within our community, case management will provide referrals to appropriate human service agencies according to the needs of our patients.

 Linda Gambone
Social Work

Linda Gambone’s expertise is in social work. She has worked in Montgomery County since 2004 when she piloted and implemented the personal navigator program. Ms. Gambone was recognized for the partnership award for outstanding services from the Montgomery County Commissioners for her work with older adults of the county. Education includes human service degree, course work in social work, and certificate in gerontology from Montgomery County Community College and Cabrini College.
