Reach for the stars! That’s what we did in establishing this exciting healthcare delivery system. Now generous donors can have their own star at Community Health & Dental Care.

Individuals and/or donors contributing $7,000 or
more will have the opportunity to select a room
in the Center that they wish to sponsor. In
addition to having their name outside the doorway
of a particular room, the donor will have their
name on a “star” in the lobby.
- For a donation of $10,000, a donor can sponsor
an exam room.
- For a donation of $25,000, a donor can sponsor
a nurses’ station or doctor’s office.
- For a donation of $50,000, a donor can sponsor
an office suite.
- For a donation of $75,000, a donor can sponsor
a dental suite.

Gifts are tax deductible to the extent provided by law. The official registration and
Financial information of Community Health and Dental Care may be obtained from the
