Who We Are

Noting a need for an accessible and affordable healthcare provider in the tri-county area – one who would serve people without insurance, people who are under-insured and people with insurance – the Pottstown Area Health and Wellness Foundation donated the seed money to establish just such a facility. That facility is Community Health & Dental Care.

To the area, we are a new way of delivering healthcare services! In a bright and cheerful setting, we provide medical, dental, and case management (previously referred to as social work services) all under one roof. Separate from us, but under the same roof, a patient has access to outpatient mental health and drug and alcohol services through Creative Health Services. Truly this community-based center, modeled after a Federally Qualified Health Center Look-Alike, is a cost effective system for delivering affordable, accessible and quality services.

In addition to our services being affordable and accessible, our staff being qualified and certified, this not-for-profit Center has taken some additional steps in transforming the healthcare delivery system and making it better.

We are using information technology. The patients’ records are part of an electronic system that enables us to maintain medical, dental, and case management records without paper. Prescriptions will also be written electronically.

CHDC also stresses “wellness” through its educational programs. We believe the power of working in partnership with the community can be key in reducing dental disease, fighting obesity, hypertension, diabetes, cholesterol, asthma and the influence of alcohol, tobacco, and illegal substances.
